What You'll Do

  • Develop individualized Wellness Coordination and Risk Plans.
  • Coordinate healthcare services and collaborate with physicians, providers, and interdisciplinary teams.
  • Conduct in-home wellness visits and health assessments.
  • Monitor health outcomes and promote preventive care.
  • Train and support Direct Support Professionals on individual health and risk plans.
  • Oversee medication administration competency and compliance.
  • Maintain accurate documentation and ensure compliance with Indiana state guidelines.
  • Participate in Individualized Support Team meetings and advocate for the health and wellness of the individuals you serve.

What You'll Bring

  • Current Indiana Registered Nurse (RN) license.
  • CPR/First Aid certification (required before your first active day of employment).
  • Excellent communication, organization, and time management skills.
  • Reliable transportation, valid driver's license, and current automobile insurance.
  • Experience supporting individuals with intellectual and developmental disabilities is preferred but not required.
