Job description

PACT RN Position at Space Center

The PACT RN is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Duties of the position include, but are not limited to:

  • Responsible for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
  • Administer medications and procedures per established policies and guidelines.
  • Influence care outcomes by collaborating with members of the interdisciplinary team.
  • Active participation in the unit/program level quality improvement process and initiatives as well as customer service programs.
  • Provide competent, evidence-based care to primary care patients and oversight of licensed vocational nurses/nursing assistants (as appropriate).
  • Provide direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. Promotion of health, health education and coaching on wellness, disease prevention, and chronic care management.
  • Direct the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization.
  • Demonstrate leadership in delivering and improving holistic, primary care through collaborative strategies with others.
  • Evaluate practice in an ongoing process, based on best evidence.
  • Provide peers with informal constructive feedback for improvement.
  • Foster a safe and supportive environment conducive to the professional development of healthcare professionals.
  • Contribute professional outpatient nursing perspective in discussions with the interdisciplinary team.
  • Partner with others to effect change and produce optimal outcomes.
  • Support colleagues through knowledge sharing to provide safe, quality outpatient nursing care.
  • Shares educational findings, experiences, and ideas with peers.
  • Participates in activities and strategies to sustain evidence-based-practice.
  • Other duties as assigned.

Licensure: 1 full and unrestricted license from any US State or territory
