Miami Jewish Health is one of the largest providers of healthcare and living options for aging adults in the Southeast. We operate Florida PACE (Program of All-Inclusive Care for the Elderly) Centers, the largest PACE program in Florida, which serves as health plan, healthcare provider, and social center designed for older adults with complex medical needs. We have four adult day health centers that serve participants in Miami-Dade and Broward counties.

Job Title: LPN PACE

Job Summary

Under supervision of the Registered Nurse, provides nursing care to assigned participants.

Essential Job Functions

Performs care for assigned participants per established department guidelines, policies and procedures, MD orders and/or coordinates under the direction of an RN

Administers medications and initiates and maintains IV therapy according to nursing rules and procedures

Counsels or teaches patients/families as planned by RN

Communicates patient care data to other caregivers according to guidelines of reporting

Documentation is kept according to established standards

Actively participates as a member of the Interdisciplinary Team

Performs departmental activities in compliance with department, regulatory, and professional standards, and guidelines

Participates in department meetings, in-services and trainings as needed

Job Requirements

Education:

High School Diploma or equivalent.

Graduate of an accredited school of practical nursing

Experience:

One of year of experience working as an LPN

Licenses/Certifications:

Current licensure by the Florida State Board of Nursing

Valid Florida Driver's License & Proof of Insurance

BLS Certification required

IV Certification preferred

Abilities Required

Must be able to work effectively in a team environment

Ability to treat patients of all ages including geriatrics

Bilingual - English and Spanish, Creole a plus

Functional Demands

Environment Work Conditions:

Normal working condition with adequate lighting and ventilation

Exposure to toxic or caustic chemicals; bio-hazardous waste; flammable, explosive gas

Ability to stand or walk 90% of the day.

Ability to bend and stoop frequently.

Ability to do frequent lifting and turning of patients/residents, singularly and with other staff members and equipment

Infectious Material Exposure:

Routine exposure to blood/body fluids/tissues
