Key Responsibilities
  • Collaborate with interdisciplinary teams, including healthcare providers, chaplains, and volunteers, to design and implement individualized care plans that address the unique emotional and psychological needs of hospice patients and their families.
  • Conduct thorough assessments to evaluate the social, emotional, and environmental factors influencing patients’ well-being and devise appropriate interventions based on these evaluations.
  • Utilize evidence-based practices to provide direct counseling and emotional support to patients and their families, helping them cope with grief, loss, and change.
  • Continuously educate patients and families about resources available to them, equipping them with the tools necessary for navigating the complexities of hospice care.
  • Stay current with developments in social work and hospice care practices to ensure the highest standard of service delivery.
Patient Population / Client Focus

Your primary focus will be on older adults and their families experiencing terminal illness. Given the sensitive nature of hospice care, you will work closely with patients to understand their unique life stories, values, and preferences to ensure dignity in their end-of-life care. This role demands empathy, patience, and active listening to truly support individuals in their most challenging times.

Qualifications
  • A Master's degree in Social Work from an accredited institution.
  • Current and active LCSW license in the state of Nevada.
  • Strong foundational knowledge of social work practices, especially within the hospice and palliative care contexts.
  • Exceptional interpersonal communication skills, allowing for effective interaction with clients, families, and multidisciplinary team members.
Preferred Experience
  • A minimum of two years of relevant clinical experience, including direct work with elderly populations and end-of-life care.
  • Proven track record in conducting comprehensive assessments and crafting actionable care plans.
