Responsibilities
Hybrid schedule:
Facility based: The RN will visit members at assisted living / independent living facilities to perform hands on assessments with the possibility of wound care and simple nursing tasks and at nursing homes to implement member's plan of care
Remote based: The RN will complete paperwork and assessments from home
Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPN's who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.
In this role, you will utilize clinical assessments performed by MJHS Med PC providers of members' medical, psychosocial, physical, and spiritual needs to implement member's plan of care.
  • In close collaboration with assigned provider, the RN case manager develops, implements, and monitors care plan, and ensures continuity of care across all healthcare settings
  • Ensures that interdisciplinary team (NP/PA, PCP, member/member's representatives, and specialists) are involved in care planning process
  • Utilizes benefit structure and community-based resources to meet member's needs
  • RN case manager is required to be on site in ISNP / IE-SNP partnered facilities
Qualifications
  • RN BSN, Masters Degree preferred
  • Minimum two years clinical work in long term care or community settings servicing geriatric population,
    managed care or home care experience preferred
  • Working knowledge of Medicare and Medicaid regulations
  • Prior experience in an interdisciplinary service delivery environment preferred.
