Job description

Duties:
Job Summary:

Functions as the clinical resource for patients and staff for ostomy, continence disorders, acute and chronic wound and skin breakdown, and fistulae. Provides direct wound and ostomy care in the inpatient and outpatient setting. Provides patient/family teaching and instruction of wound and ostomy management both pre and post-operatively. Assists the healthcare team in the evaluation of skin and wound products and equipment along with providing educational programs, in-services, and bedside teaching for the nursing staff. Stays up-to-date with skin care practices and shares advances in wound and skin care assessments, evaluations, treatments, and prevention measures.
Job Responsibilities:
Assesses all patients with a pressure injury staged at Three (3) or above.
Sees all patients that have developed a hospital-acquired pressure injury (HAPI) and meets appropriate reporting requirements.
Assesses overall hospital needs and practices to eliminate HAPIs.
