Job description
- Promote hospice mission, goals and philosophy.
- Demonstrate commitment, professional growth and competency.
- Demonstrate compliance with policies, procedures, regulations defining scope of practice.
- Promote security and safety of Protected Health Information.
- Participate in programs and on committees as requested.
- Participate in ongoing education as required.
- Provide services according to the Hospice Aide Care Plan.
- Coordinate with RN-develop/update plan of care to meet the needs.
- Attend IDG meetings, as requested.
- Document services and submit timely.
- Other duties as assigned.
- Report changes to the RN.
- State Driver’s License.
- At least one year preferred, as a hospice aide or nursing assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.
- Strong communication and interpersonal skills; Sensitivity to issues of loss and grief; Ability to deal effectively with the demands of the job and population serviced; Ability to work without consistent direct supervision.
- Reliable transportation.
- CPR certified; TB health screen or TB test/CXR according to policy; Hepatitis B Vaccination Consent/Declination
