Hospice Community Liaison/Marketer
Location: Mansfield

Are you an experienced hospice marketer who thrives on building strong referral relationships and making a difference for patients and families?

At Concho Hearts Hospice, our liaisons are trusted partners in the healthcare community. Weโ€™re looking for a motivated professional passionate about hospice care and skilled at developing meaningful partnerships with providers throughout the Mansfield area.

What Youโ€™ll Do:
  • Develop and maintain referral relationships with hospitals, physicians, assisted living communities, and skilled nursing facilities.
  • Grow referral sources throughout the Mansfield area.
  • Provide hospice education to healthcare providers and community partners.
  • Meet with patients and families to explain hospice services and obtain consents.
  • Coordinate closely with the clinical team to ensure seamless admissions.
  • Represent Concho Hearts Hospice as a trusted community resource.
What Weโ€™re Looking For:
  • Minimum 2 years of hospice marketing experience required.
  • Proven relationship-building and networking ability.
  • Self-motivated with strong organizational skills.
  • Comfortable presenting hospice education to providers and community groups.
  • Ability to work closely with clinical teams to coordinate patient admissions.
