Practice Transformation Specialist - Registered Nurse
Location: Commack
Schedule: Full Time
Days/Hours: M-F 40 hours
Salary Range: $95,000 - $115,000
(pay will be determined based on experience, preferred qualifications, and education)

Responsibilities
Summary: The Practice Transformation Specialist is a critical team member who supports the Stony Brook Medicine CIN clinical pathways and quality initiatives. The Practice Transformation Specialist serves as a primary contact for medical practice groups, collaborating directly with providers and office staff to identify opportunities for workflow improvement, integrating clinical and information technology resources to support quality initiatives and provide education and guidance on performance and patient outcome measures.
• Serve as the primary contact and liaison for assigned clinical practices.
• Engage with and educate network providers, nurses, practice administrators, and clinical/clerical support staff to improve clinical and operational workflow efficiency.
• Develop and lead efforts to increase provider adoption and use of point-of-care EHR-supported solutions.
• Use data analysis to identify opportunities for new clinical effectiveness initiatives.
• Identify and promote opportunities to standardize workflows to improve patient and practice outcomes.
• Support CIN efforts to continuously evaluate, define, and implement network-wide clinical initiatives and priorities.
• Ensure clinical activities are documented accurately and appropriately in the EHR.
• Travel expectations to several clinical practice sites is required, expectations are two to three times per week
• Other duties as assigned.
Qualifications
Required Qualifications
• NYS Registered Nurse (RN)
• Five (5) years of experience working in an ambulatory healthcare setting.
• Knowledge of ACO, HEDIS, and NCQA quality measures.
• Knowledge of quality improvement methods, measurement definition and analysis, team facilitation, and project management.
• Experience and demonstrated competence using electronic health records.
• Proficiency in using clinical information systems supporting population health management.
• Proficiency in using electronic health records (EHRs).
• Excellent communication, interpersonal, and problem-solving skills.
Preferred Qualifications
• Experience in Pediatric care.
• Bachelor's degree in a healthcare-related discipline, or equivalent relevant experience.
• Experience using Oracle/Cerner EHRs.
