Posting Summary
Rutgers, The State University of New Jersey, is seeking a Community Services Coordinator in the Department of Emergency Medicine within the New Jersey Medical School (NJMS).
The Community Services Coordinator for the Mobile Community Health (MCH) program provides direct community-based outreach and overdose response services while serving as the operational lead for the MCH team. This role combines community engagement, care coordination, and research implementation responsibilities.
Among the key duties of this position are the following:
  • Conducts community outreach to connect individuals with opioid treatment programs and MOUD providers.
  • Responds alongside EMS providers to 911 and overdose calls, providing care navigation, support services, and linkage to care.
  • Provides education, guidance, and navigation support to individuals with opioid use disorder.
  • Coordinates follow-up care and referrals to medical, behavioral health, and social service providers.
  • Collaborates with hospital and community partners to ensure seamless transitions from the Emergency Department to community-based care.

Minimum Education and Experience
  • Bachelor's Degree in Public Health, Human Services, Social Work, or a related field, plus three (3) years of relevant experience in coordination, patient navigation, or community-based services.
  • Equivalent experience, education and/or training may be substituted for the education requirements.
