We are offering a $500 sign on bonus!!

Family Service Association is seeking a part time (21 hours per week) Site Coordinator for our Countryside Adult Day Health Program in Raynham, MA. The Site Coordinator assists the Program Coordinator with management of day-to-day operations of the program, provides case management services, and monitors mental health status of program participants.  The hourly rate is $21 and the hours are Monday through Friday, 10:00 a.m. - 2:00 p.m. 

Duties and Responsibilities :

  • Provide case management services

  • Develop and lead groups for the participant.

  • Assist the program coordinator in referral, admission and discharge process.

  • Write progress notes quarterly.

  • Inform participants and their families/caregivers of available community services, and make referrals as indications warrant.

  • Attend monthly supervisor meetings with the Program Coordinator (Bi-weekly with LICSW if not licensed).

  • Assist, as necessary, in the delivery of other required program services.

  • Participate in the bi-annual or quarterly review of each participantโ€™s care plan as well as assisting with the development of care plans.

  • Assist Program Coordinator with the implementation of policies and procedures.

  • Assist Program Coordinator with record reviews, documentation, compliance, and implementation of policy and procedure.

  • Ability to provide services outlined in this job description at any of the three Adult Day Health sites as assigned by the Program Coordinator.

Education and Experience Requirements:

  • Bachelors Degree in Health or Human Services or a related field required.

  • At least One (1) year of recent social work experience working with disabled/elderly adults in a professional capacity is required.

  • Significant work experience with the elderly and/or physically/mentally challenged adults in a community setting preferred.

Must be able to successfully complete a MA State CORI check, CPS check and company paid, and pre-employment physical.
