Group Home for individuals with disabilities (not mental health care, transition, or recovery. This is Community Residential.)


Are you a site supervisor looking for the next opportunity?  Or an experienced DSP that is looking for that next step of growth?


Opportunity Matters is seeking a Site Supervisor for a house in Sartell, MN.  At this home lives a very high energy, fun to work with individual that likes to play basketball, walk to the park, play video games, and go places.  This site needs someone that can match that energy, be with the individual at all times, be able to step in during interfering behaviors and help support appropriate social interactions.  The person we serve requires active Supervision at all time, and especially during encounters with people in the community and/or visitors to the home.  The ideal candidate for this role is someone that can recognize antecedents, recognize when the person needs down time and space, and support the individual through behavior cycles.  The Site Supervisor will train and oversee staff so that the staff understands how best to work with the individual with consistency in following the programs and service plan.  Our Site Supervisors are assigned to one home site, but it is important that they understand how the Organization works and they will help out at other sites as needed.


The schedule is mainly 8a-4p M-F.  May be required to help cover other times and may work occasional weekend shifts if needed.

The Program Coordinator oversees one program under the direction of the Program Director. The Program Coordinator ensures compliance with all aspects of the program related to the persons served, the Organization, the employees, and property. The Program Coordinator is responsible for providing effective leadership and accountability to foster growth and development in the team of Direct Support staff. The Program Coordinator provides comprehensive personal care, guidance, and appropriate support tailored to the needs of each person.

Essential Duties 

  • Maintain knowledge of the Direct Support Professional job description, perform, train, and oversee all of the duties of that position.
  • Foster a positive living environment for persons served and a positive working environment for staff.
  • Build and maintain rapport with persons served, their support teams, staff, co-workers and Org leadership.
  • Act as an appropriate role model for persons served and staff.  
  • Actively supervise staff to ensure programs are implemented and the tasks at the site are being completed.  
  • Work with Program Director to develop, improve, and implement program planning for persons served.
  • Manage the weekly grocery shopping, plan menus, write grocery lists, adhere to budget. Ensure personal needs shopping is done.
  • Maintain a clean and sanitary site/program.
  • Communicate with Supervisor on persons served care plans and overall operations of the site/program.
  • Maintain records, documents, charts, and client information, legibly. Ensure staff are recording information in an accurate and timely manner.
  • Track money of persons served, oversee counting procedures, and secure the money at the site.
  • Immediately respond and address emergency situations.
  • Communicate with and assist other staff positively and professionally. Deliver disciplinary action if needed. Provide on-the-spot feedback.
  • Assist with and complete duties that need to be done at the site/program, following Organization policies and procedures.
  • Order medications and maintain Medication Administration Records.
  • Ensure there is adequate shift coverage at all times. This may include, but is not limited to, staying beyond a scheduled shift or covering/splitting open shifts with DSPFT and/or other Direct Support Staff.
  • To be considered for a promotion, start learning the Designated Coordinator qualifications and duties.

Work Environment and Physical Demands

The work environment may be slow or fast paced, depending on what is happening at the client home. This is an environment of constantly working with others. The noise level is generally moderate but can be loud.

  • Regularly lift up to 20 lbs and occasionally lift and move up to 50 lbs to waist height unaccompanied.
  • Alternate sitting and standing and walking for duration of shift.
  • Frequently required to walk, stand, bend, twist, or stoop.  
  • Frequently apply coordinated use of hands and fingers to grasp, feel objects, transfer small items from one hand to another, fine manipulation, and bilateral reach with arms.
  • Use of close vision, color vision, and ability to adjust focus.

Pay and Benefits

Starting pay range for this position is $21.50-22.50/hr. Overnight sleep shifts paid at minimum wage (can sleep at work on designated sleep shifts only). Hours 

Scheduled hours are Monday-Friday, 8a-4p.

Requirements:

Required Qualifications and Education (must-haves)

  1. High School Diploma or Equivalent.
  2. Obtain and maintain acceptable clearance of the DHS Background Study.
  3. Obtain and maintain OMI Medication Administration certificate within 60 days of hire.
  4. Valid U.S. Class D Driverโ€™s License with acceptable record under insurance guidelines.
  5. Ability to lead individuals we serve and display appropriate behavior as a role model, work without immediate supervision, multi-task, effectively communicate, make decisions, accept and follow change, and interact with others at all levels of the organization.
  6. Ability to effectively present information and respond to questions from Supervisors, Directors, clients, customers, and the general public. Read, Write, and Speak English fluently.
  7. Reasoning ability to solve practical problems, deal with a variety of variables in situations, interpret a variety of instructions in written, oral, or schedule form.  
  8. Ability to prepare food/meals: Understanding of ingredients, measuring sizes, following a recipe, and preparing all types of foods.
