Job description

The Case Manager for the HHAP (Homeless Housing, Assistance, and Prevention) program is responsible for providing direct support to individuals and families experiencing homelessness. This role involves conducting client assessments, developing individualized housing and support plans, and coordinating services such as housing placement, mental health care, and substance abuse treatment. The Case Manager will work closely with local agencies, landlords, and service providers to ensure clients receive comprehensive support. Additionally, the Case Manager will track client progress, maintain accurate records, and advocate for resources to help clients achieve long-term stability.

Case Manager Duties and Responsibilities

  • Accessing client needs, and developing strategies to help the client
  • Providing support and resources for clients; good working relationship with support networks, government resources, and community resources
  • Researching and referring client resources
  • Evaluating and studying programs, services, and resources for quality and client needs
  • Listening and providing help and advice to clients
  • Caring for the well-being of clients and helping a wide range of clients, including people with serious illnesses, addictions, or disabilities
  • Advocating and raising awareness on behalf of clients and the needs of the community and local services

Case Manager Requirements and Qualifications

  • Excellent written and communication skills
  • Excellent interpersonal skills with colleagues, community leaders, policymakers, and others
  • Excellent time management skills; organized and able to prioritize
  • Motivated to take on additional community involvement projects and solve problems
  • Comfortable in a fast-paced environment with multiple cases
  • Able to organize and manage large amounts of files, schedules, dates, and information
  • Self-directed and able to work without supervision
  • Empathetic and supportive with mentorship and leadership skills
  • Proficient computer skills(i.e. Google Docs/Word, PowerPoint, and Excel)
