Duties & Responsibilities
- Plans, prepares and implements processes and procedures for effective patient outreach and care coordination across the continuum.
- Provides social support by conducting health assessments, acting as a patient advocate and coordinating participation in health and social services programs or providing information on available resources.
- Assists the patient in utilizing appropriate and available resources when necessary.
- Coordinates care with physician practice personnel, AnMed Departments, and/or internal/external resources as identified.
- Assist patients in compliance with health screenings, maintaining/keeping office appointments and compliance with a course of therapy for identified chronic conditions.
- Assists in the transition of care from inpatient or ED visit, communicating with both the patient and family, and provider post-discharge.
Qualifications
- High school graduate or possess a GED.
- LPN with current SC license or CMA certification by American Association of Medical Assistants (AAMA), National Center for Competency Testing (NCCT), National Healthcare Association (NHA), or American Medical Technologist (AMT).
- Any combination of three years' health/social service experience and/or education.
- Must have a valid driver's license and reliable transportation.

- Bilingual English/Spanish or professional working proficiency in Spanish.
