What You'll Do
As a Case Manager, you'll play a key role in helping clients successfully navigate treatment and transition into recovery. Responsibilities include:
  • Provide comprehensive case management services for clients with moderate to high clinical and psychosocial needs
  • Coordinate discharge planning and transitions of care to support continuity of services
  • Facilitate interdisciplinary case reviews and communicate client progress, barriers, and treatment recommendations
  • Assess client needs and assist with identifying barriers that may impact treatment, discharge, or long-term recovery
  • Connect clients with housing, employment, transportation, insurance, benefits, healthcare, community resources, and recovery supports
  • Assist clients with obtaining identification, benefits, and other resources needed to support stability and independence
  • Build collaborative relationships with referral sources, community agencies, probation officers, courts, employers, healthcare providers, and other community partners
  • Advocate for clients while promoting accountability, self-sufficiency, and recovery
  • Coordinate referrals and follow up with outside providers to ensure appropriate linkage to services
  • Maintain timely, accurate, and compliant documentation within the Electronic Health Record (EHR)
  • Participate in multidisciplinary treatment team meetings, quality improvement initiatives, and compliance activities
  • Maintain appropriate professional boundaries and protect client confidentiality
  • Support a positive, structured, trauma-informed, and recovery-focused treatment environment
  • Perform other duties as assigned

Requirements
Required Qualifications
  • High school diploma or equivalent required
  • Minimum of one (1) year of experience providing case management, care coordination, or direct services in behavioral health, substance use disorder treatment, mental health, healthcare, or a related human services setting
  • Knowledge of community resources and social service systems
  • Experience documenting services in an Electronic Health Record (EHR)
  • Strong communication, organization, documentation, and follow-through skills
  • Ability to work effectively as part of an interdisciplinary treatment team
  • Ability to maintain appropriate confidentiality and professional boundaries
  • Current CPR/First Aid certification or ability to obtain within 30 days of hire

Preferred Qualifications
  • Associate's or bachelor's degree in Social Work, Psychology, Counseling, Human Services, Behavioral Health, or a related field
  • Two (2)+ years of experience in residential or outpatient substance use disorder or behavioral health treatment
  • Experience with discharge planning and transitions of care
  • Training in Motivational Interviewing, Trauma-Informed Care, or Recovery-Oriented Systems of Care
  • CADAC, Indiana Certified Recovery Specialist (CRS), Certified Peer Recovery Coach (CPRC), Certified Case Manager (CCM), or another relevant behavioral health certification
  • Knowledge of Indiana Division of Mental Health and Addiction (DMHA) standards and Indiana community behavioral health resources

Work Conditions & Physical Requirements
This position is primarily based within a behavioral health treatment facility and involves regular interaction with individuals experiencing substance use disorders, mental health conditions, and complex psychosocial needs.
The role may require:
  • Extended periods of sitting, standing, walking, and computer use
  • Frequent verbal and written communication
  • Ability to work effectively in a fast-paced treatment environment
  • Occasional travel within the community to coordinate services, attend meetings, or connect clients with resources
  • Ability to respond appropriately to emotionally challenging or crisis-related situations
  • Ability to maintain professional boundaries, confidentiality, and situational awareness within a residential behavioral health environment
