Position Description

We are looking for a dedicated Nurse Practitioner for our Skilled Nursing Facility (SNF) in Wellsville, NY. This is a full-time, in-person position, Monday through Friday, 9 to 5, with no nights, no weekends, and no on-call. As the primary care and rounding provider in the facility, you will manage a panel of long-term care and geriatric patients, working independently from facility staff while collaborating on patient care.

Qualifications of the Nurse Practitioner

  • Active New York NP license in good standing
  • Experience in skilled nursing, long-term care, or geriatric primary care preferred
