Your day.

As a Hospice Chaplain you need to know how to:

  • Partners with members of the Hospice and post-acute care services teams to provide a multidisciplinary approach to care and planning.
  • Conduct comprehensive assessments/reassessments of spiritual needs for spiritual care plan, facilitate interventions and support for patients and families within required timeline.
  • Provides services to the patient/caregiver in accordance with the plan of care.
  • Participates in multidisciplinary patient rounds, care conferences, and consultations.
  • Participates as a member of the hospice IDT in the development and implementation of the patient's care plan.
  • Documents appropriately in the patient's medical record.
  • Make reasonable efforts to arrange for visits of clergy and members of other religious/spiritual organizations from the community as per patient/family request.
  • Supports ongoing partnerships with community clergy.
  • Participates in after hours and weekend on call responsibilities.
  • Participate in maintenance of proper administrative procedures for service lines and team in accordance with institutional and regulatory policies, procedures and professional practices.
  • Participates in continuous quality improvement.
  • Participates in agency wide meetings, committees and education.
  • Serves as flex chaplain in hospice (inpatient and home), home health, outpatient palliative care and other service lines as assigned.

Your experience.

We hire people, not resumes. But we also expect excellence, which is why we require:

  • Bachelor's Degree Required; 4 Units of Clinical Pastoral Education (ACPE) are required.
  • Board Certification through the Association of Professional Chaplains or the Spiritual Care Associate required within 2 years of hire date.
  • 6 months prior hospice/and or post-acute care experience required
  • Master of Divinity, Theology or related field completed is preferred; nearing completion of Master's degree accepted
  • Board Certified Chaplain through APC or SCA, preferred
  • 1 year prior hospice experience preferred.
