Job Summary: Registered nurses (or RNs) work closely with supervising physicians to assess and monitor patients' needs. They ensure that treatment and care are conducted according to the procedures of the facility, as well as administer medications and perform other related tasks.

  • AxisCare Data Input (Care Plans, Service Agreements & Assessments)
  • Clinical Quality Reviews
  • Supervisory Visits Review
  • Training for Newly hired Staff
  • Discharge Planning
  • QI Documentation Review
  • Healthcare Patients/Plans
  • Change of Conditions Recommendations
  • Provide palliative care as needed to keep the client comfortable
  • Make recommendations for devices or tools that might improve the client's quality of life

  • Coordinator services with administrator and Office Manager
  • Proud Quality Customer Services during the visit
  • Make recommendations accuracy to change of condition
  • Quarterly quality improvement documentation are complete and reviewed we documented
  • 100% of staff administering medication are Proxy Trained in every department
