Job description

Home Health Occupational Therapist (OT)

We are seeking a compassionate and motivated Home Health Occupational Therapist (OT) to join our growing team. This role provides skilled occupational therapy services to patients in their homes, focusing on improving independence, safety, and quality of life.

The Occupational Therapist evaluates patient needs, develops individualized plans of care, and delivers evidence-based treatment in accordance with physician orders, agency policies, and regulatory standards.

Key Responsibilities
  • Evaluate patients to determine functional limitations, treatment needs, and prognosis
  • Develop and implement individualized occupational therapy plans of care
  • Provide skilled occupational therapy services in the home setting
  • Address Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs)
  • Educate patients and caregivers on treatment plans, safety, and adaptive techniques
  • Recommend and assess adaptive equipment and home safety modifications
  • Reassess patient progress and modify treatment plans as needed
  • Coordinate care with the interdisciplinary healthcare team
  • Participate in discharge planning and discharge patients when goals are met
  • Supervise Home Health Aides per agency policy and regulatory requirements
  • Maintain accurate, timely, and compliant clinical documentation
  • Adhere to professional, ethical, and regulatory standards of practice
Qualifications
  • Graduate of an accredited Occupational Therapy program
  • Current Occupational Therapist (OT) license in the state of practice
  • Minimum of one (1) year of Occupational Therapy experience; home health preferred
  • Knowledge of home health regulations and documentation requirements
  • Strong clinical assessment and organizational skills
  • Excellent verbal and written communication skills
  • Ability to work independently and manage a flexible schedule
  • Ability to legally work in the United States
Physical Requirements
  • Ability to travel between patient homes
  • Ability to lift, transfer, and assist patients as needed
  • Ability to stand, walk, bend, and reach during patient care
  • Ability to communicate effectively with patients and healthcare team members
