Lifespark – RN Case Manager, Home Health

Salary & Schedule
Salary: $88,000–$90,000 annually (DOE)
Schedule: Full-Time, Monday–Friday, 8:00 AM–5:00 PM, with every 6th weekend rotation and 2 holidays per year
Mileage Reimbursement: $0.70/mile

Position Summary
The RN Case Manager plans, organizes, and directs home health services, including Start of Care visits, care plan development, case management, caregiver supervision, documentation, and collaboration with physicians and the care team.
