Overview:
Seeking an experienced Nurse Case Manager II to support members enrolled in Medicare and Medicaid. This 100% remote role focuses on improving health outcomes through comprehensive care management and collaboration with members, providers, and community organizations.

Responsibilities:

  • Conduct thorough assessments of membersโ€™ health and social needs.

  • Develop and implement care plans using internal and external resources.

  • Apply regulatory guidelines and company policies to ensure accurate benefits administration.

  • Advocate for members to achieve optimal, cost-effective outcomes.

Qualifications:
