• Conduct thorough assessments, utilizing biopsychosocial evaluations to identify and understand clients' social, emotional, and environmental challenges.
• Develop and customize service plans in partnership with clients, ensuring all goals and interventions are clearly defined and regularly monitored for effectiveness.
• Provide individual and family counseling that addresses complex challenges, equipping clients with strategies to overcome barriers to their mental health and stability.
• Implement crisis intervention strategies and develop risk assessment and safety plans in urgent situations to ensure client safety.
• Collaborate closely with community agencies, healthcare providers, and support groups to facilitate seamless service coordination and advocate persistently on behalf of clients.
• Engage in community outreach initiatives designed to raise awareness about available services and promote client access to our diverse offerings.
• Contribute actively to case conferences and team meetings, providing insights that improve case outcomes and participating in training sessions to enhance professional skills.

Your primary focus will be on individuals and families affected by substance use disorders, mental health challenges, and significant life transitions. You will work to empower clients to access comprehensive care, breaking down barriers and providing them with the tools needed for sustainable recovery and resilience. The ideal candidate will be adept at building rapport with diverse populations, demonstrating empathy and cultural competence in every interaction.

• A master’s degree in Social Work from an accredited program, ensuring a solid foundation in clinical practices and theories.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is required to practice and engage in therapeutic sessions.
• Knowledge of trauma-informed care principles and the ability to apply these concepts in everyday practice is essential.
• Strong communication skills, particularly in understanding and addressing the unique cultural needs of varied client demographics.
• Excellent problem-solving abilities, enabling you to think critically and adapt to the evolving needs of your patients.

• Previous experience in a social service agency, particularly within a detox or rehabilitation setting, is preferred.
• Familiarity with community resources specific to Port Saint Lucie and surrounding areas for facilitating support and referrals.
• Expertise in conducting comprehensive assessments and crisis intervention, especially related to substance abuse and mental health.
