Job Description

Will be working remote when not in the field. 80% field, 20% remote

Manages 60-80 patients per caseload.

Will be given a lap tap, cell phone and office supplies

Will be traveling to member homes, long term care facilities, physician offices, hospitals and other provider offices


Qualifications

We are looking for field case managers who have experience with geriatric populations.ย 

MUST HAVE 2 YEARS OF CASE MANAGEMENT EXPERIENCEย 

Must be willing to travel to patient's homes.ย 

Must be computer literate. * Big! -Will be asked if they can operate Windows based systems.

701B Comprehensive Assessment Certificate is strongly preferred! - An exam that you take that allows you to conduct 701B type of assessment.ย 

IT will provided candidates with the skills and knowledge they need to succeed as a case manager or assessor working with elders and adults with disabilities.

Bachelor's degree in social work, psychology, gerontology or other human services related field or
