OASIS RN Weekend (Registered Nurse) - HomeCare

The OASIS Registered Nurse (RN) provides skilled nursing care to patients in their homes and community settings, ensuring high-quality, outcome-based, and cost-effective care. This role specializes in completing comprehensive OASIS assessments, developing individualized care plans, and collaborating with interdisciplinary teams to support positive patient outcomes. Key Responsibilities:

  • Complete comprehensive patient assessments, including accurate OASIS clinical and functional evaluations.
  • Review referrals and initiate patient and family contact.
  • Develop and implement individualized plans of care in collaboration with patients, families, physicians, and care team members.
  • Perform OASIS assessments throughout the patient's episode of care to ensure consistency and quality outcomes.
  • Provide skilled nursing visits, assess patient progress, and evaluate responses to treatment.
  • Educate patients and caregivers on disease management, treatment plans, and health promotion.
  • Coordinate care with physicians, therapists, and other healthcare professionals.
  • Supervise LPNs and Home Care Aides in carrying out the established plan of care.
  • Maintain accurate and timely clinical documentation and reporting requirements.
  • Participate in care conferences, interdisciplinary team meetings, SBAR reports, and quality improvement initiatives.

Required:

  • Graduate of an Associate Degree or Diploma Nursing Program.
  • Current Registered Nurse (RN) license in the State of Connecticut.
  • Minimum of one year of nursing experience or one year in a specialty nursing area.
  • Strong computer skills.

Preferred:

  • Bachelor of Science in Nursing (BSN) from an NLN-approved program.
  • Medical-surgical and/or home health experience.

Schedule: Part-time (2 - 39 hours)
