Job description

Job Summary:
The 24-hour Emergency Assistance Service provides on-call counseling, problem solving and immediate response for people living independently that need support due to a self-identified emergency. The number of people served by a 24-hour Emergency Team and the number of hours worked each week varies based on the individualized needs of the people served. Each member of the team will share responsibility for carrying the 24-hour phone for shifts that last approximately 4 days. While carrying the phone, it is expected that all calls are answered, and all responsibilities of the position are carried out.
Supervisory Responsibilities:
  • None
Duties/Responsibilities:
  • Immediate response to a person who calls or texts the 24-hour phone line
  • Adherence to a pre-determined schedule that reflects job sharing responsibilities among team members with answering calls
  • In-person visits to a person's home in emergent situations
  • Calls made to 911 or county crisis lines when person needs immediate medical intervention or mental health crisis assistance
  • Compliance with training requirements reflected in individualized services plans
  • Communication with other 24-hour emergency assistance team members to help provide seamless services when transferring phone responsibilities
  • Updates to a person's team members (case managers, guardians and extended team members) in a timely manner that reflect both positives and concerns as needed
  • Participation in monthly team meetings
  • Periodic in-person assessments of clients. Each client requires an annual visit
  • Accurate and timely documentation in various information collection platforms that reflect information about the person when contact occurs.
  • Participation and completion of training assigned by the Independence Services Director. This includes annual training on trauma and crisis response
  • Communication to people receiving the service regarding resources and referrals as needed
Requirements
Required Skills/Abilities:
  • Excellent verbal and written communication skills
  • Strong analytical and problem-solving skills
  • Ability to prioritize tasks
  • Ability to function well in a high-paced and at times stressful environment
  • Proficient with Microsoft Office Suite
  • Ability to learn new software as needed
  • Compassion and empathy towards others when they are in a crisis situation
  • Ability to work independently
Education and Experience:
  • Ability to pass a background check
  • Possession of a valid Driver's license
  • High school diploma or equivalent
  • Required at least 1 year experience working with people who have disabilities and/or mental illness and/or chemical dependency
  • Preferred 3 years' experience in the social services field

Compensation:
This is a salaried position at $15,000 annually with required driving compensated through a mileage rate of .56/mile
