We are seeking a knowledgeable, compassionate, and detail-oriented Wound Care Nurse to lead our wound management program and help improve the quality of life for our residents through evidence-based wound prevention and treatment.

The Wound Care Nurse plays a critical role in promoting healing, preventing complications, reducing avoidable hospitalizations, and improving resident outcomes. Through clinical expertise, collaboration, and education, this position helps ensure every resident receives the highest standard of wound care while supporting regulatory compliance and quality initiatives.

The Wound Care Nurse is responsible for the assessment, treatment, monitoring, and documentation of residents with wounds, pressure injuries, surgical incisions, skin tears, and other skin integrity concerns. This role works collaboratively with physicians, nurse practitioners, nursing staff, therapy, dietary, and administration to develop individualized treatment plans that promote healing and prevent further skin breakdown.

• Perform comprehensive wound and skin assessments upon admission and as clinically indicated.

• Develop, implement, and evaluate individualized wound care treatment plans.

• Perform or oversee wound treatments, dressing changes, and advanced wound care interventions.

• Monitor wound healing progress and modify treatment plans as appropriate.

• Identify residents at risk for skin breakdown and implement preventive interventions.

• Collaborate with physicians and advanced practice providers regarding wound management and treatment recommendations.

• Lead the facility's pressure injury prevention program.

• Conduct routine skin rounds and wound rounds.

• Ensure appropriate use of pressure-relieving mattresses, cushions, and positioning devices.

• Collaborate with Therapy and Dietary to optimize mobility, nutrition, hydration, and healing.

• Monitor compliance with repositioning schedules and skin care protocols.

• Maintain accurate, timely, and complete clinical documentation in accordance with CMS, AHCA, facility policies, and professional nursing standards.

• Ensure wound measurements, staging, photographs (when applicable), and treatment documentation are completed accurately.

• Participate in Medicare documentation, quality measures, and reimbursement initiatives related to wound care.

• Assist with regulatory surveys and prepare documentation for surveyors.

• Monitor wound prevalence and incidence trends.

• Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.

• Analyze wound outcomes and recommend improvements to clinical practice.

• Assist with infection prevention efforts related to wound management.

• Promote evidence-based practices to improve healing and reduce avoidable pressure injuries.

• Educate nursing staff on wound prevention, treatment techniques, documentation, and skin integrity best practices.

• Serve as a clinical resource for nursing staff regarding wound management.

• Assist with competency validation and ongoing staff education.

• Participate in interdisciplinary care plan meetings and resident care conferences.

• Current Florida Registered Nurse (RN) license in good standing required. LPN candidates with extensive wound care experience may be considered.

• Wound Care Certification (WCC, WOCN, CWCN, or willingness to obtain certification) preferred.

• Minimum of one year of wound care experience, preferably in skilled nursing, rehabilitation, or long-term care.

• Strong knowledge of pressure injury staging, wound healing principles, infection prevention, and evidence-based treatment practices.

• Experience with electronic medical records and clinical documentation systems.

• Excellent assessment, critical thinking, communication, and organizational skills.

• Ability to work collaboratively within an interdisciplinary team.

• Current CPR certification required.
