Homecare - LPN (Full Time)
Territory: Ravenna Ohio
Business Hours: Monday - Friday, 8:00 AM to 4:30 PM

The responsibilities of our Nurses include:
  • Assist the RN in carrying out the plan of care
  • Assist the RN in performing specialized procedures as directed by the RN under physician's orders
  • Assist with the preparation, implementation, and continuing evaluation of the patient care plan
  • Help teach the patient appropriate self-care techniques
  • Prepare clinical progress notes
  • Assist with the rehabilitation of patients according to the medical care plan
  • All other duties as assigned

Qualifications:
  • Graduation from an accredited School of Practical Nursing
  • Must be licensed in the State of Ohio as a LPN
  • One or more years in home health experience is preferred
  • Must have a criminal background check
  • Must have a current CPR (BLS) certification

Job Conditions:
  • Valid driver's license required
  • Ability to access patients' homes, which may not be wheelchair accessible.
  • Sufficient hearing, eyesight, and physical dexterity for patient assessments and care
  • Standard nursing activities that include walking, sitting, stooping, standing, and varying lifting/moving/turning of patients
