Job description

Job Title: Home Health Aide - Nursing Student

Location: ElderONE / Home Care

Hours Per Week: Per Diem

Schedule: Sunday through Saturday - Day and evening shifts available. Must be available at a minimum to work: 1 - 8 hour shift every 2 weeks with 1 weekend shift per month

As a nursing student you gain great patient care experience. You will provide care to clients, following their plan of care. You make a positive difference each and every day by demonstrating care and compassion to our patients.

ELDERONE

An affiliate of Rochester Regional Health, a Program of All-Inclusive Care for the Elderly (PACE). Servicing Monroe, Ontario and Wayne counties, ElderONE is an innovative alternative to long-term nursing home placement. As the area's only PACE program, ElderONE represents the future of senior health care. This long-term care program provides the entire continuum of medical care and support services to seniors 55 and older with chronic care needs, allowing them to maintain their independence in the comfort of their home without compromising their health or safety.

RESPONSIBILITIES

Helps to provide a comprehensive plan to meet patient/participant needs, daily as well as long term.
Maintains records of patient/participant daily activities, including general condition of client.
Aids in patient/participant daily routine, activities include but are not limited to, personal care, light housekeeping, and meal preparations.
Accurately and timely completes aide activity sheets, submitting activity sheets on a timely basis as specified by department policies.
Attends a minimum of 12 hours of annual in-service education/competencies
Communicates any concerns/changes in patient/participant condition to appropriate staff in a timely manner and documents on Aide Activity sheet.
Keeps family and staff informed of clients activities and concerns.
Performs other duties as require.

REQUIRED QUALIFICATIONS:
Nursing student who has documented evidence of successful completion of course work requiring mastery of home health aide tasks within the past 24 months. Official transcript will be acceptable to verify Fundamentals in Nursing.


Required LICENSURE/CERTIFICATION:
Must have valid NYS Driver's license for Home Care (preferred by ElderONE)
Must complete both written and one day HHA Skills competency program provided by Rochester Regional Health and earn active New York State HHA certification within 4 days from hire.

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
