Job description

Position Description The Nashville Strong Babies (NSB) project is designed to provide pre and postnatal care, case management and home-visiting services for approximately 700 women, infants and men in Nashville and Davidson County annually. The primary goal of the project is to reduce the disparities in infant deaths that exist across racial and ethnic groups in our community. If this idea sounds good to you, then you need to apply to become a Case Management Specialist (Program Specialist 2 - Healthy Start) at the Metro Nashville Public Health Department (MPHD).This position works with the program's high-risk participants and their families and reports to a Public Health Nurse 2, Case Management Lead. Typical Duties Manages assigned caseloads of high-risk participants with a high degree of responsibility and expectations, using specialized and technical skill sets and advanced problem-solving experience to assist clients in navigating the NSB program. Maintains direct contact with NSB enrollees and potential enrollees in the target area; includes conducting daily visit encounters (in-person) and documenting encounters in a timely manner

Escalates any notable changes (medium risk to high risk) in participant status to Care Support Lead. Uses evidence-based home-visiting curricula to deliver education on a variety of topics from prenatal through parenting. Follows up with high risk NSB enrollees to connect them to services as needed, documents service received and the outcome of the service; reports to the Case Management Lead.

Performs general administrative duties including maintaining accurate documentation of client records, periodic assessments, statistical reports, data entry, emails and letters. Acts as member of the NSB care team, attending weekly case consults with the Care Support Lead and other NSB staff. Minimum Qualifications High School Diploma or GED may be considered.

Three (3) years of paid, full-time employment in social work, pre and postnatal care, or a directly-related field. Valid driver's license, use of personal vehicle, and maintenance of valid personal vehicle insurance as required by Tennessee Law. MPHD seeks to be a socially and culturally responsive organization that acknowledges and addresses historical and current inequities in our public health promotion and practice.

Successful candidates will demonstrate the ability to work with multicultural communities. Candidates with accreditations earned in a foreign institute are encouraged to apply, however at the present time MPHD is not sponsoring individuals through any of the US Government's employment-based immigration programs. Preferred Experience, Knowledge, Skills, and Abilities Bachelor's Degree strongly preferred At least 1 year of home-visiting experience with the target population.
