JOB SUMMARY:
 
Following a collaborative team model, the Nurse Navigator identifies and manages high risk patient populations, coordinates their care, and provides safe, age-appropriate patient care. Provides community-based care navigation, providing education and information to high-utilizer patients, working with the health care team of a nurse, social worker and community health worker.  Navigator works with this team and the patient's physician to develop and implement a plan of care.
  • Development of individual, multidisciplinary plan of care for each individual patient in coordination with the physician and other health care team members.  
  • Navigation assistance Assisting patients in navigating through health care and community support systems to achieve quality care and follow up.  
  • Contact includes in-person visits in hospital, Emergency Department, physician offices, patients' homes, and over the phone.   Advocating with or on behalf of patients, families, and caregivers to address their needs or for policies and programs that will benefit them.
  • Completion of comprehensive nursing assessment to determine patient and family strengths and needs relative to effectively managing the disease process.  
  • Ensure that the agencies and institution are responsive to the needs of individual patient, families, and caregivers by conducting data collection, assessment and reporting for quality improvement and program growth.  
  • Participate with multidisciplinary team in the development of programs and resources to address the needs of high risk patient.  
  • Program development Assists in the development of the community patient navigation program and other duties as assigned.

REQUIREMENTS:

  • Minimum of 1 year of experience required; 3+ years of experience preferred
  • Current/Valid RN License in MD (compact state as applicable)
  • BSN or higher preferred
  • BLS Certification from the American Heart Association is required
