Job description

Nurse Practitioner - Family Medicine Care, Northside

We are seeking an Advanced Practice Provider to join one of the nation's most integrated healthcare systems, with over 1,300 providers in Indianapolis, IN. This opportunity is located at Family Medicine Care, Northside 7525 E. 82nd Street, Suite A, Indianapolis, IN 46256.

Community Health Network has an excellent opportunity for a family medicine trained advanced practice provider to work at our outpatient Family Medicine Care, Northside location.

  • Single, robust electronic medical record system (EPIC)
  • Competitive compensation and benefits package

Minimum of 3–5 years of experience as an Advanced Practice Provider in a primary care setting.

Must successfully complete a nationally recognized Advanced Practice Nurse certification examination or have current national certification; must maintain certification on an ongoing basis.

Must be eligible for prescriptive authority under the criteria outlined by the Indiana State Board of Nursing; must maintain eligibility for prescriptive authority on an ongoing basis.
