We are seeking a driven and relationship-focused Hospital Liaison to support admissions growth for a respected long-term care company operating multiple skilled nursing and rehabilitation facilities throughout the region.
This position is responsible for developing and maintaining strong referral relationships with hospitals, physicians, case managers, discharge planners, and healthcare partners across the North County market. The ideal candidate will be highly motivated, professional, and passionate about helping patients transition successfully into post-acute and long-term care services.

Position Summary
The Hospital Liaison is responsible for increasing census and strengthening referral partnerships for multiple skilled nursing and rehabilitation centers. This role focuses on building trust with referral sources, educating families and healthcare professionals on facility services, and coordinating smooth transitions of care from hospitals to post-acute settings.

Key Responsibilities
Develop and maintain referral relationships with hospitals, physicians, case managers, discharge planners, and other healthcare professionals
Increase admissions and overall census for multiple skilled nursing facilities
Educate referral sources, patients, and families on rehabilitation, skilled nursing, and long-term care services
Serve as a liaison between hospitals and facility admissions teams to coordinate patient transitions
Maintain strong knowledge of facility services, clinical capabilities, specialty programs, and payer sources
Participate in daily and weekly census development and admissions meetings
Collaborate closely with facility administrators, nursing leadership, and admissions teams
Identify opportunities for market growth and new referral development
Represent the organization professionally within the healthcare community and at networking events
Conduct regular facility visits and maintain visibility within assigned hospitals and referral accounts
Track referral activity and maintain accurate documentation and communication
Qualifications
Bachelorโs Degree in Marketing, Healthcare Administration, Business, or related field preferred
Minimum of two (2) years of healthcare marketing, admissions, liaison, or medical sales experience preferred
Skilled nursing facility (SNF), rehabilitation, long-term care, or post-acute care experience strongly preferred
Existing relationships within the North County healthcare market preferred
Proven success building hospital and physician referral relationships
Knowledge of Medicare, Medicaid, managed care, and other payer sources preferred
Strong communication, presentation, and interpersonal skills
Highly organized, self-motivated, and goal-oriented
Professional appearance, behavior, and communication required
Ability to work independently while collaborating with multiple facilities and teams
