Are you a compassionate Registered Nurse eager to make a lasting impact on the lives of first-time mothers and their families? The Children & Family Health Institute (CFHI) is seeking a dedicated Home Visitor RN to deliver essential care and support through the Nurse-Family Partnership (NFP) program in Monmouth County, New Jersey. This role offers a unique opportunity to guide families toward healthier pregnancies, positive child development, and brighter futures.

CFHI delivers high-impact programs including nutrition support, preventive screenings, vaccinations, and inclusive health services through community health centers and mobile outreach. Our goal is to strengthen scale and stability while advancing a mission that creates meaningful impact in the community.
As a Home Visitor RN, you will:
  • Conduct in-home visits to provide nursing services aligned with the NFP model and Visit-to-Visit guidelines, managing a caseload of up to 25 clients
  • Assess and address the health and developmental needs of women and their families
  • Offer counseling and instruction to help families achieve targeted health and life goals
  • Build and maintain strong community relationships to facilitate client referrals and resource connections
  • Attend weekly team meetings, case conferences, and clinical supervision sessions
  • Mentor and precept new hires and support student nurses during home visits as needed
  • Ensure compliance with agency infection control and safety policies
  • Maintain professionalism in adherence to organizational policies on absenteeism and appearance

Qualifications
  • Associate's degree in Nursing required
  • 2-3 years of relevant nursing experience; home visiting or maternal/child health experience preferred
  • Proficiency in computer use, excellent verbal and written communication, strong organizational and interpersonal abilities
  • A commitment to CFHI's mission and the ability to build trusting relationships with clients and colleagues
  • Bilingual in English and Spanish preferred
