Job Description

Located in Martin County

-Will be working remote when not in the field

-Will be in the field depending on the week 50-80% of time

-Will be required to go into the local office in area probably about once a month.

-This role is a clinical RN position on a specialized LTC team which focuses on the transition of members from skilled nursing facilities to home and community based settings.

-Will be doing post charge follow up

-Will be accessing charts, patients and homes assessments for safety and appropriateness.

-Will be required - 4 members per week total of 12 per month

Qualifications

-RN

-3+ years of clinical nursing would prefer from an acute can setting

-Candidate MUST have discharge planning/post discharge follow up experience.

-Can come from ANY setting, preferably from Managed Care.

-Need to be computer literateย 

-Would Prefer Home Health or LTC experience

-Make sure candidates have utilized Community Resources they are working on getting these people out of the Nursing Home!

Hours for this Position:
M-F: 8 hour shift between 8:30am-7:00pm
