???? What You’ll Do
  • Conduct comprehensive in-home patient assessments
  • Develop and update individualized care plans
  • Manage medications, wound care, and chronic disease support
  • Educate patients and families on self-care and health management
  • Coordinate care with physicians and interdisciplinary teams
  • Supervise and support LPNs and Home Health Aides
  • Ensure accurate, timely documentation of patient progress
  • Monitor and adjust care based on patient condition changes

What We’re Looking For
  • Active Registered Nurse (RN) license
  • Prefer a minimum of 2 years of nursing experience (Home Health preferred)
  • Strong clinical judgment and assessment skills
  • Ability to work independently in the field
  • Excellent communication and patient education skills
  • Valid driver’s license, reliable transportation, and auto insurance
  • Current CPR certification
  • Experience with HomeCare HomeBase preferred (not required)
