Job Requirements / Qualifications
  • Current, active Registered Nurse (RN) license (state of practice or multi-state compact)

  • Perform comprehensive nursing assessments on all patients within 8 hours of admission and develop individualized care plans
  • Collaborate with interdisciplinary teams and families to identify patient teaching needs and measurable goals
  • Provide timely, accurate documentation of assessments, interventions, and patient responses in the electronic medical record
  • Participate actively in patient rehabilitation programs and discharge planning from day one
  • Communicate changes in patient status to providers to ensure continuity of care and patient safety
  • Supervise and provide direction to LPNs, CNAs, and orientees; support clinical development of teammates
  • Promote and maintain a safe environment for patients and staff, adhering to hospital policies and LTAC standards
  • Assume charge responsibilities as needed following orientation and demonstrate leadership on night shifts
  • Engage in ongoing professional development and continuing education relevant to LTAC nursing and travel RN roles
