Salary: $39.90/Hour | Full-timeย 

Schedule: Friday 5p - Monday 8a

Must have Home Health or Hospice Experience

As a Registered Nurse (RN) | in home health and hospice you will be called to care when you are needed most. As part of Interim Healthcare Home Health, you will support a full range of patient services to bring comfort and dignity to our clients. This is a full-time position working as an on-call nurse between the hours of Friday 5:00pm to Monday 8:00 am.

As a Registered Nurse (RN: ย 

Provide in-home direct patient care according to the providerโ€™s prescribed plan of care (and nursing scope of practice) while closely monitoring progress/recovery ย 

  • Assess and document patient conditions and report change as needed (vitals, blood/stool/urine, glucose, etc.) ย 
  • Administer medication, insulin, and IV/fluids, documenting thoroughly ย 
  • Inspect and care for wounds, changing dressings and assisting with personal hygiene ย 
  • Assess patient for further injuries, infections, bedsores, etc. and deliver appropriate treatments ย 
  • Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes ย 
  • Participate in ongoing interdisciplinary assessment of the patient
  • Participates in Quality Assessment and Performance Improvement (QAPI) program
  • Supervises paraprofessional and licensed professional team members

To qualify for a Registered Nurse (RN): ย 

  • Graduate of an accredited school of nursing and has a current license as a Registered Nurse in CA
  • Two (2) years of current nursing experience preferred
  • Must have Home Health or Hospice experience
  • Community Health and a wide range of Medical Surgical experience preferred, not required
  • Ability to pass a Criminal Background Check, Physical, TB & Drug Screen
  • Must have a current Driver's License and reliable transportation
