Hartford HealthCare Independence at Home enables elderly & disabled individuals to achieve maximum independence, to participate in their own plan of care, and to live with dignity while receiving quality care in their own homes. Our staff of highly trained professionals treat patients holistically, considering family and living situations for the most appropriate care plan.
The Homemaker Caregiver position provides:
• Personal hygiene and grooming care to clients
• Assistance with toileting, ambulation, etc.
• Providing meals for clients' nutritional needs
• Reporting changes in clients' condition to nurse, supervisor, or client care coordinator
• Performing routine housekeeping and home management duties
• Recording each client's care on care plan and submitting care plan in a timely manner
• Assisting with providing emotional support for the clients and their families
• Assisting with routine therapy exercises
• Participating in case conferences as requested
• Attends all required in-service education sessions
• Has the ability to use computer software program and smart phone for required education
• Represents the company in a positive and professional manner through a high level of customer service
• Can think critically without direct supervision and the ability to escalate when appropriate.
Qualifications:
Hourly Caregivers must have a driver's license and car with auto insurance
One year of previous personal care experience at a facility or agency
Two years of continuous work experience
New Grad CNA's are welcome
Caregivers must be comfortable driving within 30 minutes of where they live for cases.
