As a Family Support Coordinator (FSC), you will play an essential role in guiding families through the delicate process of organ, tissue, and eye donation. Through your support, you will be instrumental in helping families make informed decisions, offering compassion and understanding every step of the way.Scheduled 24-hour on-call shifts is an essential function of this role. Candidates must remain available to respond as needed during assigned on-call periods and be able to travel extensively throughout Network for Hope's service area.Key Responsibilities:Referral ManagementRespond independently to hospital referrals within designated timelines, ensuring timely communication.Report relevant patient information to the Administrator on Call (AOC) for further coordination.Confirm donor eligibility in collaboration with the AOC.Collaborate with the healthcare team to assess the readiness of the legal next of kin or family for donation conversations.Donation ConversationLead empathetic, compassionate discussions with families experiencing sudden loss, offering a safe space for grief and emotions.Build rapport with the healthcare team to gather necessary information about family dynamics, determining the best time for donation discussions.Advocate for donation with sensitivity, tailoring your approach to each family's unique circumstances.Assist families in understanding the grave prognosis.Donor Family ManagementConduct uniform Donor Risk Assessment Interviews (DRAIs) with family members to assess suitability for donation.Provide families with accurate, timely information regarding the donation process, collaborating with medical and hospital staff as needed.Act as a liaison between the family and the healthcare team, ensuring their needs are met and supporting their wishes.Follow up with families after the recovery process to inform them about donation outcomes.Family SupportProvide continuous emotional support to families throughout the donation process.Educate families about the donation process, helping them understand each step clearly and compassionately.Support families and help them prepare for their departure from the hospital.Offer memory-making services and connect families to community resources through NFH-led initiatives.Maintain regular communication with NFH staff, ensuring coordinated and consistent support throughout the process.DocumentationObtain and document authorization for donation.Maintain accurate, detailed records of family interactions, dynamics, and healthcare team communications.Follow established documentation protocols to ensure accurate, thorough case details.Other DutiesEducate hospital staff about the authorization, care process, and donation procedures. Extensive experience and healthcare certifications may substitute for education requirements and will be reviewed on a case-by-case basis.Experience: Experience in marketing, communication, teaching, health care, counseling, pastoral career other public service-related required.
