Job Summary:
The Transplant Social Worker acts as a liaison between families, hospital staff, and patients. The Social Worker plans, implements and facilitates social service interventions for Nephrology, Advanced Heart Failure, Ventricular Assist Device, and Solid Organ Transplant patients across the full continuum of care. Provides psychosocial assessment and ongoing updates to address patients' evolving needs. Works with patients to access and secure resources necessary to receive and maintain a ventricular assist device or solid organ transplant.
EXPERIENCE DESCRIPTION:
Minimum 2 years experience working in health care is required. Experience working with clients with advanced disease is preferred. Solid Organ Transplant and/or ambulatory medical social work experience is preferred. Knowledge of end stage disease processes beneficial.
EDUCATION DESCRIPTION:
Masters Degree in Social Work with casework in medical social work is required.
LICENSURE DESCRIPTION:
Current State of Wisconsin certification as a social worker is required.
