What is a Neighborhood Health Center?

A Neighborhood Health Center is a community-based healthcare facility that provides primary care and preventive services to individuals and families, often in underserved or medically vulnerable areas. These centers offer services such as medical, dental, behavioral health, and sometimes pharmacy care, regardless of a patient’s ability to pay. Neighborhood Health Centers focus on improving access to quality healthcare for all, especially those who are uninsured or underinsured. They are typically funded through federal, state, and local support and may offer sliding fee scales based on income.

What are the key skills and qualifications needed to thrive as a Neighborhood Health Center professional?

To thrive as a professional in a Neighborhood Health Center, you need a background in healthcare or public health, often supported by relevant degrees or certifications such as RN, LPN, or community health worker credentials. Familiarity with electronic health records (EHR) systems, patient management software, and telehealth tools is typically required. Strong interpersonal skills, cultural competence, and the ability to communicate effectively with diverse populations help build trust and support patient engagement. These skills and qualifications are essential for delivering accessible, patient-centered care and improving community health outcomes.

What are some common challenges faced by professionals working at a Neighborhood Health Center?

Professionals at Neighborhood Health Centers often encounter challenges such as managing a high patient volume, addressing diverse and sometimes complex health needs, and working with limited resources. These centers typically serve underserved communities, which can mean handling language barriers, cultural differences, and patients with limited access to healthcare. However, staff also benefit from strong teamwork and the rewarding experience of making a tangible difference in community health.

Job description

Registered Nurse - Community Health

If you believe healthcare is a right, that everyone deserves high quality care so they can enjoy their highest level of health and wellbeing, and you value each person's individual story – consider joining us at Neighborhood!

We are seeking a Registered Nurse who is passionate about providing high quality primary healthcare to high risk populations, including those in our Healthcare for the Homeless program. Do you know case/care management principles, do you find joy in working with patients with various backgrounds, and have an understanding of social drivers of health? This Monday-Friday role may be for you.

About the Role:

As the Registered Nurse - Community Health you'll use your training, critical thinking, and kindness as you consider patient cases, identify solutions or suggest alternate approaches to patient problems. Much of your interaction with patients will be over the phone, although you are open to meeting patients where they are and engaging patients in a variety of settings. You'll be part of a clinical team that collaborates to design and implement individualized patient-centered care. Essential duties include:

  • Initiate care plans, and manage/coordinate care; identify and address barriers affecting patients' ability to adhere to healthcare plans
  • Build trusting relationships with patients to facilitate access to health care services and meet health needs
  • Assist patients and families with navigating Neighborhood's services as well as outside community resources, programs or services
  • Attend community events as an Registered Nurse to provide health education
  • Participate in some evening street/shelter outreach, getting to know potential patients, assessing needs, and encouraging them to establish care

You will be primarily based at our Mattina location, 300 Niagara Street in Buffalo, and will also travel to other Neighborhood sites on a regular basis.

What it's Like to Work at Neighborhood:

The top three words employees say describe the work environment are: teamwork, supportive, kind. These are from an anonymous survey of Neighborhood employees for the Buffalo Business First Best Places to Work competition. Neighborhood has earned "finalist" distinction in the competition the last four years. We are a group of flexible and kind individuals who are open to each other's ideas, and see opportunities to innovate and find solutions when challenges arise.

Education and Experience needed:

  • Degree from accredited two year nursing school
  • Current NYS Professional Registered Nurse license
  • 2+ years clinical nursing experience
  • CPR certification
  • Knowledge of case/care management principles
  • Attention to detail, organizational skills, project and time management skills, and ability to follow through
  • Kindness: you treat each person with respect and compassion, valuing each person's story
  • Resiliency: you see opportunities to innovate and find solutions when challenges arise
  • Teamwork: you are open to others' unique perspectives, and will collaborate to meet shared goals
  • Knowledge of Microsoft Office Suite, Adobe, and database management

What We Offer:

Compensation starts at $36.50 per hour.

Benefits: You'll have options for medical, dental, life, and supplemental insurance. We also offer a 403b match, health savings accounts with employer contribution, wellbeing programs, continuing education opportunities, generous paid time off and holidays.

Registered Nurse Tuition Reimbursement Potential: Our status as a Federally Qualified Health Center (FQHC) means that you have an opportunity for tuition reimbursement up to 60% repayment of qualified student loans directly related to a nursing degree. Nurses who are granted reimbursement are required to fulfill a two year, full time commitment to maintain eligibility. The reimbursement program requires an application and is not guaranteed.

About Neighborhood: Neighborhood Health Center is the largest and longest serving Federally Qualified Health Center in Western New York, and is the highest ranked health center for quality in the region. We provide primary and integrated healthcare services all under one roof, regardless of a person's ability to pay. Services include internal/family medicine, pediatrics, OB-GYN, dentistry, podiatry, psychiatry, vision care, nutrition and behavioral health counseling, and pharmacy services. We're working toward a Western New York where all enjoy their highest level of health and wellbeing.
