Job description

Job Title: Family Reentry Support and Help (FRESH) Care Manager
Location: Wadesboro, NC-HYBRID
Hours: 40 hours/week, Full Time, M-F, Exempt
Coastal Horizons Center, Inc. is seeking a qualified Care Management professional for
our Family Reentry Support and Help (FRESH) program. We have this specific opportunity located in:
Anson County, NC (Polkton, NC)
Primary duties include providing case management and related services focused on the child-parent relationship and the re-entry/transition process for incarcerated parents and their children, both within correctional settings and in the community. This is a hybrid position; the FRESH Care Manager works at the prison, specifically Anson Women's Correctional Facility, and from home.
REQUIRED EDUCATIONAL/EXPERIENTIAL QUALIFICATIONS:
Education: Associate's degrees with 4 years' experience or Bachelor's with 2 years'
Certifications: Preference is given to individuals with North Carolina Peer Support Specialist certification and a minimum of 2 years of experience, and/or individuals registered with the North Carolina Addiction Specialist Professional Practice Board (NCASPPB).
Experience: One (1) year of experience working with individuals, families, and children involved in a correctional setting and the community.
Additional Skills: Ability to maintain a flexible work schedule, including travel. Participate in and successfully complete any required training and background check provided by the NC Department of Adult Correction (NCDAC). Participate in and successfully complete virtual training for Parenting Inside Out (PIO). Proficient computer skills, including the ability to navigate the internet, word processing and spreadsheet applications, and basic email platforms. Bilingual Spanish/English speaking skills are a plus!
