Addiction Social Worker (LCSW / LMSW) - Mineola
At $75,000 to $85,000 across our New York City and Long Island clinics, we are looking for an Addiction Social Worker who has been in the work and knows how to meet people where they are. You will deliver assessments, counseling, and care coordination for patients living with addiction, drawing on evidence-based, trauma-informed practices and connecting them to treatment and recovery support. Alongside our CASAC counselors and peer advocates, and with AI tools handling the paperwork, your time stays where it matters, with the patient.

What You'll Do
  • Assess and screen. Conduct psychosocial and substance use assessments and complete OASAS LOCADTR level of care determinations.
  • Counsel and intervene. Provide individual and group counseling using Motivational Interviewing, CBT, and other evidence-based modalities.
  • Support medication-assisted treatment. Coordinate care for patients on MAT alongside prescribing providers.
  • Coordinate and connect. Build recovery-focused care plans and link patients to treatment, harm reduction, housing, and community resources.
  • Partner with the SUD team. Work closely with CASAC counselors and peer advocates to keep patients engaged in care.
  • Document to standard. Keep accurate, timely records that meet OASAS and New York State requirements, with AI copilots assisting.

Requirements
  • Master's Degree in Social Work (MSW) from an accredited institution
  • Active New York State license as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
  • Direct experience treating or supporting patients with substance use disorders is required
  • Familiarity with evidence-based SUD practices: Motivational Interviewing, Cognitive Behavioral Therapy, the OASAS LOCADTR level of care determination, and care for patients on Medication Assisted Treatment (MAT)
  • Ability to deliver care that is strength-based, trauma-informed, and tailored to each patient's recovery goals
  • Strong knowledge of New York community resources, Medicaid/Medicare, and recovery support systems
  • Bilingual fluency in Spanish is highly preferred
