Job description

GENERAL DESCRIPTION:
The Patient Transportation Driver is responsible for transporting authorized patients of MyCare Health Center to and from authorized appointments at MyCare clinics.
SPECIFIC DUTIES:
  1. Transport passengers in a safe and lawful manner. This includes the use of seat belts for all passengers as well as car seats (patient provided) for those individuals under eight years of age and less than 4 foot 9 inches tall in accordance with Michigan State law.
  2. Driver may be required to assist with storing walkers and wheelchairs.
  3. Be prompt in keeping appointments and adhering to agreed upon schedules, keeping in mind that schedules do fall behind due to unforeseen events.
  4. Abide by MyCare’s rules of confidentiality.
  5. Record time and mileage using the assigned forms. Turn in these forms in a timely manner so that files are kept up to date.
  6. Conduct routine van inspection to ensure patient and employee safety and bring any defect/damage immediately to the attention of the supervisor.
  7. Report as soon as possible any problems or concerns regarding a specific client or transportation request.
  8. Work with the and any other team member to best meet the needs of the client.
  9. Treat clients with respect and dignity.
  10. To perform only those transportation requests authorized by MyCare staff. Assignments may include medical and dental appointments, substance abuse, counseling or group sessions. Driver may be asked to assist with Board member transportation to Board meetings.
  11. Maintain company vehicle, as directed by the Medical Practice Manager, such as frequent washing, vacuuming and required service visits (oil changes, tire rotations). Maintain service log.
  12. As directed by a supervisor, performs other related and/or necessary tasks to achieve organizational and programmatic goals and objectives.
  13. Responsible for compliance with all MyCare policies and procedures.
  14. Promote efficient, organized and pleasant operations in the health center and in the community, consistent with the mission of MyCare to offer care in an atmosphere of dignity and respect.
  15. Upholds the mission of MyCare Health Center by providing integrated, affordable and quality health care to those who need it most
KNOWLEDGE, SKILLS AND ABILITIES:
  1. Ability to maintain confidentiality in all matters.
  2. Meets all attendance and punctuality requirements to ensure proper coverage and quality service.
  3. Professional and appropriate dress as required by the position.
  4. Ability to establish and maintain effective professional relationships with coworkers and healthcare providers to achieve maximal results for the practice’s patient from a system of integrated primary health care.
  5. Excellent interpersonal and customer service skills, to effectively and tactfully deal with a diverse group of patients, staff, customers, and community groups.
  6. Must be able to represent MyCare with tact and sensitivity.
  7. Must practice good time management strategies.

MINIMUM QUALIFICATIONS:
  1. Valid Michigan Chauffeurs’ Driver’s license, with favorable driving record to be re-evaluated at least every 6 months.
  2. Current auto insurance certificate.
  3. High school diploma or GED.
  4. Must pass a criminal background check.
  5. Submit to pre-hire and annual Fitness for Duty Health exam attesting to no medical limitations in ability to perform duties.
  6. Submit to pre-hire and intermittent unannounced urine drug screens indicating no illicit or illegal drug use. Drug screens must be completed within 48 hours of request.
  7. CPR certification offered; not required.

While performing the duties of this Job, the employee is regularly required to sit; use hands to finger, handle, or feel. Must be able to talk or hear. The employee is occasionally required to stand; walk and reach with hands and arms. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, color vision and ability to adjust focus.
