Job description

MyCare Medical is actively seeking a full-time, certified Nurse Practitioner/Physician Assistant is the primary clinical provider for MyCare At Home patients. The Advance Practice Provider is responsible for the home visits, TCM visits, medication optimization, standardized assessments, patient self-scheduling, and care team coordination. Our company is physician-founded, and dyad-led. We’ve created a structural support system to ensure that our providers can focus solely on patient care.

About the Role

Responsibilities

  • Can work part-time in the clinic and part-time seeing patients at home.
  • Complete and submit all encounter plans within 72 hours of each visit ending
  • Perform comprehensive medication management using the R.O.C. framework (Reconcile, Optimize, Comply) at every visit
  • Administer all required standardized clinical screenings: PHQ-2/9, Mini-Cog, STEADII, REALM-SF, and Zarit Caregiver Interview
  • Escalate clinical needs to VMH Physician and/or Social Worker via patient case forward in EClinicalWorks
  • Self-schedule the next visit before leaving each appointment; write next visit date on the Patient Instruction Sheet and place it on the patient’s refrigerator
  • Conduct telephonic outreach between visits as clinically indicated
  • Perform acute care visits as clinically warranted to prevent unnecessary ED utilization

Qualifications

  • Certification through the American Academy of Nurse Practitioners (AANP) OR American Nurses Credentialing Center (ANCC)
  • Certification through the American Academy of Nurse Practitioners (AANP) OR American Nurses Credentialing Center (ANCC) program, focusing on adult medicine
  • Certification through an accredited PA certification program focusing on adult medicine
  • Currently licensed as an Acute, Adult/Gerontological or Family Nurse Practitioner in the state of practice or the ability to obtain a license prior to employment
  • Ideal Candidate, with a minimum of two years of experience as Nurse Practitioner
  • Bilingual English Spanish speaking preferred
  • Holds or is eligible for DEA licensure and CDS licensure (if required in the state of practice)
  • Up-to-date CPR Certification and Basic Cardiac Life Support certification

Required Skills

  • Strong organizational and prioritization skills
  • Ability to use Electronic Medical Record (EMR) system
  • 2 years of experience of Medical Advantage, HEIDS, MRA and ICD-10 Coding
  • Comfortable managing a primarily adult-aged patient population
  • Demonstrated ability to problem-solve complex and multifaceted situations
