Administrative Case Manager Job DescriptionPosition Summary

The Administrative Case Manager provides comprehensive administrative case management services in accordance with Single County Authority (SCA) program requirements and DDAP regulations. This role supports individuals with substance use and behavioral health needs through care coordination, referrals, documentation, client engagement, and linkage to community resources. The Administrative Case Manager works collaboratively with providers, physicians, and community stakeholders to ensure clients receive timely and appropriate services.

Essential Duties and Responsibilities
  • Provide Administrative Case Management services in accordance with SCA program requirements.
  • Maintain assigned caseload and ensure all documentation is accurate, complete, and compliant with regulatory standards.
  • Maintain at least 60% of work time in direct case management services.
  • Communicate effectively with clients, treatment providers, referral sources, and community stakeholders regarding case progress and service coordination.
  • Coordinate care, referrals, and inpatient bed placements for clients in need of treatment services.
  • Conduct collateral outreach activities to support client engagement and continuity of care.
  • Link clients to community and supportive resources including Medicaid (COMPASS), Chesco Connect, housing programs, CRS services, and other case management supports.
  • Assist physicians with scheduling appointments, assessments, documentation, and prior authorization processes.
  • Manage transportation systems and client transportation resources, including Lyft accounts and related scheduling.
  • Complete PA WITS data entry accurately and within required timelines.
  • Ensure compliance with DDAP regulations, SCA requirements, confidentiality standards, and agency policies.
  • Attend supervision sessions, staff meetings, and all required trainings.
  • Perform other related duties as assigned.
Qualifications and Education Requirements
  • Must meet DDAP MET requirements.
  • Bachelor's degree in a related field with at least one (1) year of relevant experience, or an equivalent combination of education and experience.
  • Completion of required confidentiality, Motivational Interviewing (MI), and DDAP trainings.
  • Knowledge of ASAM criteria, substance use disorder systems, and applicable regulations.

  • Strong organizational and time management skills.
  • Ability to multitask and prioritize responsibilities in a fast-paced environment.
  • Excellent verbal and written communication skills.
  • Proficiency with EMR systems and PA WITS data entry.
