Joining our team means more than taking a job-you're directly supporting some of our community's most vulnerable members and helping them navigate their healthcare journey.Position OverviewThe Medical Case Manager (MCM) is a key member of the Medically Vulnerable Population (MVP) team, providing patient-centered case management for unhoused and medically vulnerable individuals. The MCM conducts needs assessments, develops personalized care plans, coordinates medical and behavioral health services, and facilitates access to supportive resources.
