For nearly 35 years, Fourth Street Clinic has provided essential health care services to individuals experiencing homelessness in the Salt Lake City area. Joining our team means more than taking a job-you're directly supporting some of our community's most vulnerable members and helping them navigate their healthcare journey.

Position Overview

The Medical Case Manager (MCM) is a key member of the Medically Vulnerable Population (MVP) team, providing patient-centered case management for unhoused and medically vulnerable individuals. The MCM conducts needs assessments, develops personalized care plans, coordinates medical and behavioral health services, and facilitates access to supportive resources. This role ensures continuity of care, addresses social determinants of health, and advocates for patients to achieve optimal health outcomes.

Key Responsibilities

  • Provide medical case management for a designated caseload of 25–34 patients

  • Conduct thorough needs assessments for medically vulnerable patients

  • Develop and update individualized, patient-centered care plans

  • Coordinate and facilitate delivery of prescribed medical and supportive services

  • Arrange transportation for patients to access services both within the clinic and with community partners

  • Collaborate with interdisciplinary teams and external service providers to ensure continuity of care

  • Maintain accurate patient documentation and meet medical/data reporting requirements

  • Stay current on clinic policies, procedures, systems, and electronic health records

  • Participate in weekly check-ins with the MVP Clinic Manager

  • Support a healthy work environment and uphold FSC's values, fostering respect, trust, and collaboration

Required Qualifications

  • High School Diploma or equivalent (Bachelor's degree in a healthcare-related field preferred)

  • Experience in medical case management or related patient support functions (preferred: 2+ years in ambulatory care)

  • Strong attention to detail and commitment to accuracy in all work

  • Ability to work collaboratively and demonstrate a caring, compassionate approach to patient care

Preferred Qualifications

  • Bilingual in English and Spanish

  • Experience working with medically vulnerable or unhoused populations
