Brookside Multicare Center located in Smithtown, NYย  is seeking C.N.A's for all shifts (6a-2p, 2p-10p, 10p-6a). All position will be every other weekend. And is an 1199 Union position.ย 

Below is a description of what the position entails:

Department:ย  Pedatric Department

Job Title:ย Certified Nursing Assistant

REPORTS TO:ย ย Nurse Manager/District Nurse

COORDINATES WITH:ย  All Departments

Education/Experience/QUALIFICATIONS

  • Current New York State certification.ย  Listed in the New York State Nurse Aide Registry.
  • Two years nursing home experience preferred.

Physical/Mental Qualifications

  • Must be physically and mentally able to meet the needs of the residents in their care.
  • Must be able to lift and transfer residents with or without mechanical lifts
  • Must be able to assist in the evacuation of residents.
  • Must be able to lift, push, pull and move a minimum of fifty pounds (50 lbs.)
  • Good physical health, muscle strength and dexterity.ย  Good vision and hearing with or without corrective aides.
  • Must be able to speak and write the English Language in an understandable manner.
  • Must be able to maintain a professional demeanor when under pressure.

SCOPE:ย Provides direct personal care and services to residents including,

but not limited to, safety, comfort and personal hygiene.ย  May provide protection services to residents under the supervision of a Registered Professional Nurse or Licensed Practical Nurse.

Duties and RESPONSIBILITIES

1.ย ย ย  Bathe, toilet and dress residents.ย  Combs hair, gives mouth and nail care and gives performs general resident grooming.*

2.ย ย ย  Assists with meal service including passing of trays and helping to feed residents.ย  Assists residents in the dining room and collects trays.*

3.ย ย ย  Distributes nourishments when indicated.*

4.ย ย ย  Makes beds and is responsible for complete care of residentsโ€™ unit and personal possessions.*

5.ย ย ย  Assists in the maintaining a clean unit, including, but not limited to, utility room, nursesโ€™ station, pantry, day room and shower room.*

6.ย ย ย  Take vital signs, weights, measures intake and output, enemas and collects some specimens, test some specimens and report all results to the district nurse.

7.ย ย ย  Report any and all changes in residentsโ€™ conditions to the District Nurse and/or Nurse Manager.*

8.ย ย ย  Document all pertinent information pertaining to resident care, including but not limited to, Nurse Aide Accountability Records, Activity of Daily Living Forms, vital signs and weights.*

9.ย ย ย  Assists with ambulation, lifting, transferring residents, range of motion exercises, while maintaining proper body alignment and providing a safe environment. Transports residents throughout the facility.*

10. Responsible for giving report of residentsโ€™ daily activities and nursing services rendered to the District Nurse/Relieving Nurse Aide and/or Nurse Manager before going off duty for break or end of shift.*

11. Follows acceptable infection control practices including standard/universal precautions.*

12. Provides a safe environment for the residents by monitoring day room activities and implementing restorative accommodations as directed by the Nurse Manager or District Nurse. *

13. Attends and completes all mandatory inservices required, including 12 hours of inservice to maintain certification.*

14. Perform other related duties as required.
