• Conduct comprehensive assessments to evaluate the mental health status of patients, integrating tools like the Diagnostic and Statistical Manual of Mental Disorders to propose targeted interventions.

• Develop and continually adapt individualized treatment plans based on patient progress, cultural backgrounds, and unique needs, ensuring alignment with therapeutic objectives.

• Utilize advanced electronic health record systems to prepare and maintain accurate clinical documentation, ensuring compliance with regulatory standards and internal protocols.

• Collaborate effectively with patients and relevant stakeholders to create comprehensive care plans that reflect desired treatment outcomes and foster a supportive therapeutic alliance.

• Provide periodic evaluations and adjustments to treatment approaches, with a focus on enhancing patient safety and symptom management, demonstrating a responsive care model.

• Facilitate referrals and coordinate with treatment providers, ensuring continuity of care through established professional relationships with various clinicians and community resources.

As a Licensed Clinical Social Worker in our community health clinic, you will address complex issues such as anxiety, depression, trauma, and grief, tailoring your approach to sensitive, patient-led care. • A Master’s degree in Social Work (MSW) from an accredited institution.

• A current and valid LCSW license in the state of California.

• Proficiency in clinical assessment tools and documentation best practices in accordance with regulatory requirements.

• Ability to perform physical tasks relevant to client care, such as standing, walking, and lifting items up to 25 pounds as needed.

• A minimum of two years of direct clinical experience in a mental health setting.

• Familiarity with community resources and therapies applicable to diverse populations.

• Experience with telehealth practices and electronic health record systems will be advantageous.
