Job description

With over 23 programs, we offer multiple ways to aid seniors maintain their health, well-being, independence and participation in the community, fulfilling our mission.

Under the supervision of the Practicum Field Instructor and Program Supervisor, MSW Interns participate in the provision of care management services and coordination of supportive services for low-income older adults and people with disabilities. This is a paid, educational internship designed to support the development of clinical and case management skills in a supervised learning environment.
Interns will gain hands-on experience in assessment, care planning, and service coordination while working closely with experienced care managers.

Key Responsibilities

  • In partnership with the Field Instructor, develop a comprehensive learning agreement to establish educational goals and objectives
  • Complete required onboarding, including Workday training, documentation training, and shadowing visits
  • Participate in a structured learning experience that includes:
    • First-year MSW interns: Co-manage a caseload of approximately 10 clients alongside a Care Manager mentor
    • Second-year MSW interns: Co-manage a caseload of approximately 12-15 clients alongside a Care Manager mentor
  • Under supervision, assist in conducting comprehensive client assessments, including psychosocial, physical, mental health, environmental, and spiritual needs
  • Support the development and implementation of client-centered care plans (Community Living Plans)
  • Assist with identifying, coordinating, and monitoring appropriate community-based services
  • Learn and apply knowledge of Medicare, Medi-Cal, and other entitlement programs relevant to the population served
  • Build and maintain professional relationships with clients, caregivers, and service providers while demonstrating respect, dignity, and professionalism
  • Participate in client visits (minimum monthly or as appropriate), with guidance from supervisor
  • Learn and follow agency documentation standards, including timely and accurate progress notes (within 48 hours)
  • Maintain organized and accurate records to support continuity of care
  • Support client and caregiver education and empowerment strategies
  • Participate in evaluating the effectiveness of interventions
  • Assist with case transitions, including closure or transfer at the end of the internship
  • Participate in interdisciplinary team (MDT) meetings, staff meetings, and trainings as assigned
  • Maintain regular communication with supervisors and actively engage in supervision
  • Complete all required academic documentation in collaboration with Field Instructor
  • Adhere to the ethical standards of the National Association of Social Workers (NASW)

Education Requirements

  • Bachelor's degree (BA or BS)
  • Seeking a MSW internship program from accredited MSW program.

Preferred Qualifications

  • Experience working with older adults and/or individuals with disabilities (preferred)
  • Familiarity with medical and psychosocial needs of vulnerable populations (preferred)
  • Strong communication, organizational, and problem-solving skills
  • Ability to manage multiple tasks and prioritize effectively
  • Basic computer proficiency (e.g., documentation systems, Microsoft Office)

Physical & Work Requirements

  • Ability to perform essential job functions with or without reasonable accommodation
  • Ability to conduct field-based work, including traveling to client locations
  • Ability to use standard office equipment and complete electronic documentation

Language Requirements

  • Ability to communicate effectively in English for client care, documentation, and collaboration with care teams
  • Additional language skills are a plus

Compensation

  • $19.61 per hour (San Francisco minimum wage; subject to applicable updates)

Work Environment & Safety

This role includes field-based work in the community. The organization provides guidance and training to support safe practices during client visits.
